Request Copies of Your Medical Records
Complete the Authorization for Release of Information form and submit it using one of the following methods:

We will provide records within 10 business days per NYS law. Most requests are completed within 72 hours.

How do I request copies of my medical records?
Complete the Authorization for Release of Information form and submit it by fax to 315-476-3505, by email to HIM@syracusecommunityhealth.org, or by dropping it off at any of our locations.
What information should I include in my request?
Please include your full name, date of birth, dates of service (if known), what you are requesting, where to send it, and your signature.
How long will it take to receive my records?
We provide records within 10 business days, as required by New York State law. Most requests are completed within 72 hours.
How will I receive my records?
Delivery options depend on your request and what is permitted by law. Please contact the HIM Department if you have a preferred delivery method.
Do I need to show ID?
We may ask for identification to protect your privacy and ensure your records are released only to the appropriate person.
Can someone else request my records on my behalf?
Yes. With your written authorization, records can be released to another person or organization. The Authorization for Release of Information form must be completed and signed by you or your legal representative.
How do I sign up for the patient portal (MyChart)?
MyChart lets you securely view recent office visits and lab results online. Sign up to access your health information anytime. Contact us if you need help getting started.
