Our comprehensive services include primary care, dental care, behavioral health, eye care, pediatrics, women’s health, QuickCare, addiction services, podiatry, care management, and family support services.
We welcome patients of all ages, regardless of insurance status or ability to pay. Medicaid, Medicare, commercial insurance, and self-pay patients are welcome, with sliding fee discounts available for those who qualify.

Welcome to Syracuse Community Health. Growing up in a community with similar struggles to those in Syracuse, my passion has always been to deliver high-quality, equitable care to those who need it most.
We believe quality healthcare is a right, not a privilege. As a Federally Qualified Health Center, our goal is to be your one-stop shop for integrated primary care, dental, and behavioral health, all under one roof. Yes! We offer secure telehealth appointments for many services. Ask when scheduling.
What languages do you offer services in?
We provide translation services in multiple languages to serve our diverse community.

On Wednesday, KeyBank and the Key Bank Foundation presented Syracuse Community Health with the $300,000 Thrive Together family wellness initiative grant.
At Syracuse Community Health, this investment has allowed us to expand proactive outreach health initiatives and enhanced focus on infants and maternal that are transforming how we care for our patients. Key Bank Foundation didn’t simply fund a program. It has enabled us to launch a new model of care—one that brings together medical care, behavioral health, and social support.
This means developing new pathways and a population health department identifying those patients who have fallen out of care, reconnecting and re-engaging individuals to close gaps in preventive services and help patients better manage complex chronic conditions before they become medical emergencies.
We are beyond grateful for your generous donation, your partnership, and your investment in the health of our community. Thank you!
